01 Your DD-214

Any version of it. A photo on your phone is fine. We are looking at dates of service and anything already documented in it.

02 Your most recent VA rating decision letter

If you do not have it saved: log in at va.gov, go to My VA, select Manage all claims and appeals, then All of your claims and appeals. The decision letters live there.

03 Every condition you are dealing with, and when it started

Including the ones you have never filed for. Sleep, knees, back, hearing, breathing, mental health. Roughly when each one started matters as much as the condition itself.

04 Who is treating you now, and what you are taking

VA, private, or both. Providers and current medications tell us what evidence already exists and what still has to be built.
